The tibialis posterior tendon curves behind the medial malleolus and fans out to the navicular and midfoot, acting as the chief dynamic support of the medial longitudinal arch. Its degeneration or rupture is the most common cause of adult-acquired flatfoot deformity. Maintaining its strength is key for arch stability in runners and load-bearing athletes.
